We’re seeking a Support Worker to join our dedicated Modern Slavery Transitional Housing Program, including the Safe House. You’ll play a vital role in helping clients secure stable housing, develop independent living skills, and reconnect with their communities. Reporting to Program Manager this is Max term, part time (22.8 hours per week) position. Salary is in accordance with SCHADS Level 4 As a Support Worker, you will: Manage Transitional Housing Program referrals by conducting assessments to determine eligibility, manage waitlist, ascertain appropriate alternative supports and make referrals as needed. Effectively manage case load as allocated by the Program Manager Provide individualised, client-led case management from intake to exit. Support clients to secure and sustain safe, stable accommodation. Facilitate referrals and connect clients to essential services and community supports. Build relationships that promote self-determination, resilience, and recovery. Work collaboratively with internal teams and external partners, including Salvos Housing. Participate in shared on-call duties to support Safe House residents. Service gaps are identified and recommendations for service delivery improvements are made to the Team Leader/Program Manager Promotes program and implement strategies to ensure program objectives are met Work collaboratively with other service providers, representing the organisation in relevant network and interagency meetings and working groups as directed. About You Certificate IV in Social Work, Community Services, or similar (or relevant experience). Strong understanding of housing support, trauma-informed care, and client-centred practice. Excellent communication, advocacy, and organisational skills. A team player who can manage a caseload with compassion, professionalism, and cultural sensitivity.
