We are seeking a dedicated and caring person to join our team at 3 of Anglicare's villages in Woollahra, Glebe and Paddington. You will provide a high level of customer service to residents and their families, to build rapport with residents and support them to age in place, and to also assist in welcoming and onboarding new residents into village life. Your Role: In this role, you will be a cornerstone in understanding and addressing the care needs of our older residents. You'll help them navigate the community aged care options available through My Aged Care, ensuring they receive the compassionate care and support they need. Why You'll Love this Job: Undertake health and wellbeing assessments for new and existing residents Build rapport with residents to identify Home Care services that best meets their care needs. Support residents through the My Aged Care process to obtain assessments and referral codes to access home care services. This includes referrals and advocacy as required. The Health and Wellness Coordinator will also work closely with the Anglicare at Home team to meet the social, wellbeing and health needs of our residents.
