Care Finder Adviser Community Services

We are seeking an care finder Adviser to join our team. The position is offered as a Full-time fixed term contract until 30th June 2026.

The role aims to deliver specialist and intensive assistance to help people understand and access aged care and connect with other relevant support.  The successful applicant should have the skill and ability to build and leverage a local network. Aged Care Alternatives has been contracted by the Adelaide PHN to deliver care finder services in the Adelaide metropolitan region.

The Role:

• Undertake assertive outreach
• Support clients to interact with My Aged care so they can be assessed
• Assist them to find aged care and community supports and services

Essential Criteria:

• Certificate IV in case management or client care coordination or similar qualification
• Experience in assessment, referral and creation/coordination of human services
• Current Driver's licence (essential)
